1. The Provincial Government of Misamis Oriental, through the MOPH-Manticao-Drugs & Medicines intends to apply the sum of SIX HUNDRED FIVE THOUSAND SIX HUNDRED EIGHTY-NINE PESOS (Php605, 689.00) being the ABC to payments under the contract for the Supply and Delivery of the following:

1. aminophylline 25mg/ml, 10ml 10 amps

2. amiodarone 50mg/ml, 3ml 10 amps

3. biphasic isophane human insulin 70/30, 10ml 30 vials

4. bupivacaine + dextrose 0.5% + 8%, 4ml 10 vials

5. dextrose 50%, 50ml vial 50 vials

6. furosmide 20mg/ml 100 amps

7. gentamicin 40mg/ml, 2ml 50 amps

8. haloperidol 5mg/ml, 1ml 5 amps

9. hydralazine 20mg/ml, 1ml 30 amps
