Brief description of the subject of the order: 1. The subject of the order are materials for sterilization: sleeves, tapes, indicators, crepe papers, tests, envelopes, tray tags, bags, seals, tubes, non-woven fabrics, labels Part No. 1 for the University Clinical Hospital of Fryderyk Chopin in Rzeszow., 2. DetailsA new description of the objects of the order, indicative quantities of deliveries and requirements have been included in the Price and Offer Form for Part No. 1 (Appendix No. 1 to the SWZ), which is an integral part of the ToR, 3. The scope of the order includes the delivery of the objects of the order in the quantities specified in the Price and Offer Form dla Part No. 1 (Appendix No. 1 to the ToR), which is an integral part of the ToR, for a period of 12 months. 4. The Ordering Party allows the submission of equivalent offers. 5. Planned delivery of the items of the order for 12 months, counting from the date of conclusion of the contract. The Ordering Party allows for the possibility of changing the term of the contract in case ofDue to the failure to complete the entire subject of the contract during the period of the contract due to the implementation of lower orders, an amendment to the contract requires an annex, however, the Contracting Authority unilaterally notifies the Contractor of the proposed extension of the contract before the expiry of the period for which it was concluded.
