Tender Details

The subject of the order is: supplies of dressing materials and medical nonwoven materials for SPZOZ WSS No. 3 in Rybnik. A detailed description of the subject of the order is included in the annexes: 1) Price form (separately for each package) - Annex No. 1.1. down SWZ, 2) Designed provisions of the contract that will be introduced into the content of the contract - Annex No. 2 to the SWZ. The order is divided into 4 packages (parts). Agreements regarding the resolved packages will be concluded after the completion of these proceedings period of 24 months. The Ordering Party informs that in accordance with Art. 139 of the Public Procurement Act, it will first examine and evaluate the offers, and then subjectively qualify the Contractor whose offer was rated highest in a given package, in terms of the lack of grounds for exclusion. and meeting the conditions for participation in the proceedings. Fulfilling the obligation arising from Art. 24 section 6 of the Act of June 14, 2024 on the protection of whistleblowers, the contracting authority informs that in the Independent Public Health Care Facility of the Provincial Hospital Specialist No. 3 in Rybnik adopted a procedure for internal reporting of information about violations of the law and taking follow-up actions.
