Task No. 1: Delivery of consumables for hemodialysis Task No. 2: Delivery of sets for inserting permanent dialysis catheters Task No. 3: Delivery of a long-term, tunneled two-channel hemodialysis catheter 1.The subject of the order is: successive deliveries of consumable materials for continuous renal replacement therapy (hereinafter referred to as the goods), of the type and quantity specified in the price form constituting Annex No. 2 to the SWZ and in accordance with the draft provisions of the contract constituting Annex No. 4 to this SWZ, i.e. according to the tasks as follows: Task No. 1: Deliveries of consumables for hemodialysis, Task No. 2: Delivery of sets for inserting permanent dialysis catheters Task No. 3: Delivery of a long-term, tunneled catheter two-channel hemodialysis device in accordance with the detailed description of the subject of the order included in the Price Form constituting Annex No. 2 to the SWZ. The contractor may submit an offer for one, several or all parts. In the process of examining and evaluating offers The ordering party will consider each task separately. 2. The contractor should offer the subject of the order in accordance with the parameters specified by the Ordering Party.
