Catheters for thrombus aspiration Y-connector for coronary angioplasty procedures Intravascular ultrasound catheter Catheter for functional measurements 1. The subject of the order is: Supplies of cardiological medical materials. Split order is for 4 packages (parts). Agreements regarding the resolved packages will be concluded after the completion of these proceedings for a period of 12 months. 2. A detailed description of the subject of the order is included in the annexes: 1) Price forms (separately for each of them packages) - Annex No. 1.1 to SWZ, 2) Designed provisions of the contract that will be introduced into the content of the contract/contract provisions - Annex No. 2 to the SWZ. 3. The Ordering Party informs that in accordance with Art. 139 PZP will first examine and evaluate the offers, a then it will subjectively qualify the Contractor whose offer was rated the highest in terms of the lack of grounds for exclusion and compliance with the conditions for participation in the proceedings. 4. Fulfilling the obligation arising from Art. 24 section 6 of the Act of June 14 2024 on the protection of whistleblowers, the contracting authority informs that the Independent Public Health Care Facility, Provincial Specialist Hospital No. 3 in Rybnik, has adopted a procedure for internal reporting of information on violations of the law and taking follow-up.
