2. Certificate of Product Registration (CPR) from FDA/Proof of Renewal

3. Certificate of Good Manufacturing Practice from FDA

4. If the supplier is not the manufacturer, certification from the manufacturer that the supplier is an authorized distributor/dealer of the product/items

5. License to Operate from FDA with List of sources (whether it is manufacturer, importer, seller or distributor)

Have at least 2-3 days in the processing of CSP (Covid Meds Only)

During Delivery:

1. Winning supplier must deliver items with at least 24 months of expiry

2. Guarantee Letter for items below 24 months expiry

3. Batch Release certificate from FDA

4. Delivery Receipt/Invoice bearing Lot Nos and Expiry dates of the drugs and medicines
