The subject of the order is to provideContractor of the successive supply of drugs included in the drug program for the treatment of moderate and severe forms of plaque psoriasis and atopic dermatitis AZS and hypercholesterolemia in adults who qualify for general treatment for the 105th Kresowy Military Hospital withSP ZOZ clinic in Żary.

Task No. 1 - Delivery of the biological drug GUSELKUMAB

The contractor's duties will include the delivery of the biological drug Guselkumab covered by the drug program for the treatment of moderate and severe forms of plaque psoriasis in adults who qualifyfor general treatment in the quantities and range specified in the assortment and price form, constituting Annex No. 9 to the SWZ
