Tender Details

1. The subject of the order is the successive deliveries of disposable gloves for the needs of the University Clinical Hospital. Fryderyk Chopin in Rzeszow.

2. A detailed description of the items of the order and approximate quantities of deliveries during the period covered by the order are included in the Price Forms - oferts - Annexes No. 1 for individual parts that are an integral part of SWZ.

3. The scope of the order includes the delivery of the order items in the quantities specified in the Price and Offer Forms for a period of 24 months.

4. The Ordering Party allows solutions equivalent to those described in the SWZ

5. ORDER COMPLETION DEADLINE:

Successful delivery of the order items for 24 months from the date of conclusion of the contract.

The Ordering Party allows for the possibility of changing the contract validity period in the event of failure to complete the entire subject of the contract (basic order) during the contract period as a result ofek fulfillment of lower orders.

A change to the contract requires an annex, provided that the Ordering Party unilaterally notifies the Contractor of the proposed extension of the contract before the expiry of the period for which it was concluded.
