PART/TASK NO. 1 Physiotherapy services and neurodevelopmental therapies: Part No. 1 covers approximately 60 hours, place of service provision: Widnica city: The subject of the order is the provision of physiotherapy services, including specialized neurodevelopmental methods for children in age 0-2 years. CPV code 85142100-7 Physiotherapy services The duties of the Contractor of a given part will include: 1) diagnosing problems, 2) conducting neurodevelopmental physiotherapy, 3) keeping documentation, including: specialist's work sheet, work card with the client, 4) keep a register of admitted persons, 5) perform the activities that are the subject of the contract with due diligence, 6) supervise the correct implementation of this order, 7) provide services in accordance with the schedule, 8) submit by the 5th day of the month a summary of provided support and/or therapy, taking into account: the number of hours devoted to physiotherapy and the number of people who benefited from it. As part of the service, the Facilitator/Service Provider is obliged to take part in meetings, workshops, and possibly additional team work ordered by the Ordering Party.
