The subject of the contract are services for the performance of service: periodic inspections recommended by the manufacturer with the replacement of components and repair of medical apparatus for radiotherapy and medical apparatus for imaging diagnostics, for the University Clinical Hospital Fryderyk Chopin in Rzeszow:

A. Part 1 - service for medical apparatus for radiotherapy for the production of Varian Medical Systems:

a) accelerator Truebeam SN H194781 - 1 pc;

b) accelerator Truebeam SN H192853 - 1 pc;

c) accelerator Truebeam SN H193880 - 1 pc;

d) Simulator ACIUTY Ix SN H770377 - 1 pc;

e) RGSC SN HGS2269 system - 1 pc;

f) ARIA SN HIT2783 SYSTEM - 1 pc.

B. Part 2 - Service service for medical apparatus for diagnostic imaging of Siemens:

a) computed tomograph of Somatom Definition as 128+ - 2 pcs;

b) computed tomograph somatom go.all - 1 pc;

c) digital x -ray apparatus Luminos Drf Max - 1 pc;

d) x -ray apparatus Digital Select FD blow - 1 pc;

e) ALPHA digital x -ray apparatus - 1 pc;

f) mammograph mammomat inspiration - 1 pc;

g) Replacing the Straton x -ray lamp in the event of a failure in any of the computed tomographs of Somatom Definition as 128+;

h) Replacing the ATHLON x -ray lamp in the event of a failure in the computed tomography of the Somatom Go.all;
