SERVICE DELIVERY INDICATOR SURVEY (HEALTH) & VERTICAL INTEGRATION DIAGNOSTIC AND READINESS TOOL SURVEY IN ARMENIA
ORGANIZATION CONSULTANCY
A. PROJECT BACKGROUND AND OBJECTIVES
1. Background
As part of the broader engagement to support the design of Universal Health Coverage (UHC) reforms in Armenia, the World Bank (WB) and GAVI, the Vaccine Alliance, have responded to a request by the Ministry of Health (MOH) to provide technical assistance and recommendations for improving the quality performance of its primary health care (PHC) and integration of service delivery across tiers of the health system, with an emphasis on urban areas.
The Government of Armenia places strategic importance on the quality of the healthcare services provided at the PHC level. Given the rapid epidemiologic and demographic transitions underway and the challenges imposed by the COVID-19 pandemic, the MOH and the WB want to ensure that Armenia-s health facilities provide services of good quality and are responsive to the needs of the population they serve. Therefore, the WB is financing a Service Delivery Indicator (SDI) survey to measure quality and capacity of PHC facilities across the country. The SDI survey will not only inform the plans of the MOH, but also, will provide key insights for WB work in the country and inputs to the performance assessment of the Primary Health Care Performance Initiative (PHCPI).
SDI surveys generate nationally representative data on the quality of health services. The data collected generate a platform for social sector accountability and benchmarking of results, within and across countries, and over time. Since the inception of the program in 2010, 24 surveys have been completed in 12 countries in Africa, capturing the health and primary education service delivery experience of over 500 million people. The surveys now extend around the globe: preparation of data collection for SDI surveys in Latin America (Guatemala), East Asia/Pacific (Indonesia), Middle East/North Africa (Morocco), South Asia (Bhutan), and Europe/Central Asia (Moldova) are currently underway.
The health SDI in Armenia will measure PHC service delivery, with a special focus on measuring:
(i) Facility management, infrastructure, equipment, and supplies: Are health facilities appropriately stocked with necessary medicines and equipment? Are there gaps in facility infrastructure? Is the management applying mechanisms for tracking performance or providing supportive supervision to staff?
(ii) Health care professional-s knowledge, behavior, and satisfaction: Are health care professionals accurately diagnosing and treating common health conditions, adhering to MOH clinical guidelines? Are health care professionals seeing a high number of patients each day? Are they satisfied with their work environment?
(iii) Patient experience, satisfaction, and out-of-pocket costs: Do patients report receiving the services they need? Are they satisfied with their care? Do patients have high out-of-pocket expenses associated with traveling to the health facility and receiving care?
To assess the integration of service delivery and inform policy recommendations, the SDI survey will be complemented with the "Vertical Integration Diagnostic and Readiness Tool - A Survey Instrument for Countries Working Toward Health System Integration". This tool was developed by the Joint Learning Network for Universal Health Coverage (JLN), with support from the Bill and Melinda Gates Foundation and Results for Development. For this reason, it will be referred to as “JLN tool” from now on.
The objective of the JLN tool is to assess vertical integration, defined as the way and means in which care is deliberately and systematically coordinated, including communication and learning, across providers working at different tiers or settings of the health system (e.g., PHC facilities, hospitals, diagnostic units, and community settings) to provide appropriate, timely and high-quality care. Specifically, the assessment will focus on the Instrument 2 of the JLN tool “Vertical Integration in the Healthcare System, Organizational Environments and Front-Line Service Delivery Settings”, whose items aims to gather information on components (“what”) and practice (“how”) of vertical integration in the broader health system and organizational environments as well as front-line service delivery settings. It can also be used to assess system, organizational and front-line readiness for vertical integration initiatives. The Instruments 2 of the JLN tool is based on the conduction of interviews, whose respondents typically include: program administrators, supervisors, facility managers and front- line practitioners, including physicians, nurses, community health workers and midwives.
2. Objective
The objective of this consultancy is assessing the quality of PHC at the facility level and the integration of service delivery across tiers of the health system in Armenia. The assessment will be conducted by developing and conducting (i) an SDI survey for the health sector and ii) the Instrument 2 of the Vertical Integration Diagnostic and Readiness Tool of the JLN “Vertical Integration in the Healthcare System, Organizational Environments and Front-Line Service Delivery Settings”.
3. Scope of Work
The consultant organization will conduct an SDI survey in a nationally representative sample of health facilities using the SDI survey methodology. The sample will include approximately 250-300 primary health facilities (contingent on sampling decisions yet to be finalized), and up to 10 medical staff and 10 patients per facility, depending on the staff count and patient load in each facility. The sample will include both public and private health facilities, as well as both urban and rural settings. The facilities to be surveyed will be defined by the WB in discussion with government counterparts, and the list will be provided to the consultant organization.
The SDI survey has 3 main types of questionnaires (as described in Table 1 below). Each survey is expected to take 1.5 days on average, allowing for movements to and from facilities. There will be two visits per facility in the sample: (i) a first announced visit; and (ii) a second unannounced visit done at least 3 full working days after the announced visit.
