We invite you to participate in the procedure for the selection of an entity to carry out occupational medicine tests for Moto-Profil, Chorzów branch. Tests subject to cooperation: preliminary, periodic, control. Detailed data is included in the attachment - Request for quotation. Appendix No. 1 constitutes requirements for the service and cooperation (two tabs). Attachment No. 2 and Attachment No. 3 will be made available to Partners who sign the NDA agreement. Attachment No. 4 - constitutes the NDA agreement necessary to receive additional attachments No. 2 and 3 and to start cooperation with Moto-Profil. Form of signing the NDA agreement: traditional and electronic. Signed NDA agreements do not mean the commencement of cooperation in the field of occupational medicine research. It is not possible to submit a partial offer - such offers will not be considered by the tender committee. Please provide the net amount for 1 piece of a given type of test. Additional information: Location of the clinic/clinic at a maximum distance of 15 km from the location of Moto-Profil Chorzów, Niedźwiedziniec 10, 41-506 Chorzów • ProLogis Park • DC2 building. Working days are from Monday to Friday from 7 a.m. to 5 p.m., excluding holidays and public holidays. All tests are performed in one place (outpatient clinic/clinic) and on the same day. Free of charge for the duration of the tests. parking lot for Moto-Profil employees. IT (system) solutions in the field of schedules, documentation, communication, history, etc. are welcome.
