Description : 1. The Civil Aviation Authority of the Philippines (CAAP) through its Bids and

Awards Committee intends to uppty ttre sum of Sixty'Two Thousard Four

Hundred Pesos (Php62, 4fi).fi)) inclusive of all applicable govemment taxes, feet

and charges, being the Approved Budget for the Contract (ABC) to payments

under the contract for the project, Monthly Preaentioe Maintenance Sentices of

Mitsubisfui Eleoator at Manila CNS Tower Eacility.

2. The BAC requests Mr, Arvin Cruz, Authorized Representative of International

Elevator and Equipment, Inc. to submit a PRICE PROPOSAL inclusive of VAT. A

proposal received more than the ABC shall be automatically rejected at proposal

opening.

3. The submission shall include the following documents:

A. Eligibility Requirements:

1. Valid Mayols or Business Permit;

2. Valid PhiIGEPS Certificate of Registration;

3. Valid Certificate of Sole Distributorship; and
