REQUEST FOR QUOTATION
The Local Government Unit of Lake Wood, Zamboanga del Sur, through its Executive Services, will undertake a Small Value Procurement of Construction of Military Bunkers and Facilities at Dagum of this municipality in accordance with Section 53.9 of the Implementing Rules and Regulations of Republic Act No. 9184.

The Contract : FIFTY EIGHT THOUSAND PESOS ONLY (Ph 58, 000.00)

Delivery term : Within Seven (7) calendar days from receipt of the Notice to Proceed
Interested suppliers are required to submit their valid current Mayor-s Permit, Income/Business Tax Return, PHILGEPS Registration Number, Omnibus Sworn Statement, and price quotation form (Annex -œA-) upon submission of offer/quotation.
Award of contract shall be made to the lowest quotation, which complies with the minimum description as stated above and other terms and conditions stated in the price quotation form.
Any interlineations, erasures or overwriting shall be valid only if they are signed or initialed by the bidder or his/her duly authorized representative/s.
Submission of quotation and eligibility documents is on or before 9:00 p.m of March 23, 2020 at the Office of the Bids and Awards, Lake Wood, Zamboanga del Sur. Open submission may be submitted, manually.
