REQUEST FOR QUOTATION
Small Value Procurement- Section 53.9
The Technological University of the Philippines - Manila intends to procure Costume Garments with an Approved Budget for the Contract of Four Hundred Forty-Six Thousand Pesos (PhP 446, 000.00) for TUP-Manila Cultural Affairs Office.
In line with this, please quote your lowest responsive bid/price on the item/s listed on the attached Quotation Form (Annex -œA-). Please submit also your quotation duly signed by you or your authorized representative subject to the General Conditions specified below at the Office of the BAC Secretariat 2nd floor, Administration Bldg., Technological University of the Philippines Manila Ayala Blvd., Ermita Manila not later than February 15, 2019 of this year. Late submission will not be accepted.
General Conditions:
1. ALL ENTRIES MUST BE SIGNED BY THE BIDDER OR AUTHORIZED REPRESENTATIVE;
2. ALL SUPPORTING DOCUMENTS MUST BE CERTIFIED TRUE COPY BY THE BIDDER;
3. DELIVERY LOCATION: TUP MANILA, AYALA BLVD., ERMITA MANILA, SUPPLY OFFICE;
4. DELIVERY PERIOD: THREE (3) CALENDAR DAYS UPON RECEIPT OF NOTICE TO PROCEED/PURCHASED ORDER;
5. WARRANTY SHALL BE FOR A PERIOD OF SIX (6) MONTHS FOR SUPPLIES & MATERIALS, ONE (1) YEAR FOR EQUIPMENT FROM DATE OF ACCEPTANCE BY THE PROCURING ENTITY;
6. PRICE VALIDITY SHALL BE FOR A PERIOD OF ONE HUNDRED TWENTY (120) CALENDAR DAYS; AND
7. THE SUPPLIER WITH LOWEST QUOTATION SHALL SUBMIT THE XEROS COPY OF FOLLOWING DOCUMENTS, TO WIT:
a. SEC/DTI Registration;
b. Business/Mayor-s Permit;
c. BIR Registration Certificate;
d. Tax Clearance; and
e. PhilGEPS Registration Number or Certificate
8. PLEASE SUBMIT YOUR QUOTATION IN SEALED ENVELOPE and Signed.
9. THIS UNIVERSITY RESERVES THE RIGHT TO REJECT ANY OR ALL BIDS, TO WAIVE ANY INFORMATION HEREIN OR TO ACCEPT SUCH BIDS AS MAY CONSIDER MOST ADVANTAGEOUS TO THE UNIVERSITY.
