The contractor is obliged to submit: a) an offer form in accordance with the template of Attachment No. 2 b) an assortment and price form in accordance with the template of Annex No. 2A c) technical data sheet of the product, confirming the parameters indicated in points 2a-2d of the OPZ (Annex 1). All documents submitted by the Contractor as part of the execution of the order must be prepared in Polish. In case documents prepared in a foreign language, the Contractor is obliged to submit a translation into Polish, certified by the Contractor. This procedure is conducted in accordance with the Regulations for awarding public procurement contracts with a value below PLN 170, 000.00 net, constituting Annex No. 1 to the Management Board Resolution No. 170/2025 of December 15, 2025. The contractor may submit only one offer in electronic form, with a valuation of the subject of the order described in the request for quotation and observing the specified deadline. Failure to meet any of the above. conditions will result in the offer being rejected.
