1. The subject of the order is the supply of a critical parameters analyzer - 1 set. 2. The main subject of the order according to the Common Procurement Vocabulary (CPV): 33100000-1 Medical devices, 38434520-7 Blood analyzers 3. A detailed description of the subject of the order has been specified in Annex 1 to the Request for Quotation. 4. The Ordering Party does not allow the submission of partial offers. 5. Scope of activities and tasks of the Contractor as part of the performance of the subject of this contract: a) Delivery (transport, shipment insurance) of the subject of the contract the registered office of the Ordering Party in Dęblin, ul. Szpitalna 2; b) Installation and commissioning of the subject of the contract in accordance with the offer; c) Training of the Ordering Party's employees in the use of the supplied equipment. Bringing the staff to a state of knowledge that will allow for safe use of the equipment and the use of all its parameters and its operation in accordance with the manufacturer's recommendations. The training is to be completed with the issuance of certificates confirming the acquisition of the necessary skills in given scope; d) Providing the Ordering Party, together with the delivery, with an operating manual prepared in Polish for the apparatus and equipment that is the subject of the contract, and the manual should be in the form of an original company publication of the manufacturer or authorized representative; e) The Contractor is obliged to provide device passports along with the delivery of the equipment.
