The Contractor is obliged to submit: - form offer in accordance with the template of Annex No. 2; - assortment and price form in accordance with the template of Annex No. 2A; - Together with the offer, the Contractor is obliged to submit the document(s) indicated in point 9 of the OPZ (Appendix No. 1), i.e.: catalog card and/or technical data product and/or other document confirming the parameters indicated in point 2a of the OPZ. All documents submitted by the Contractor as part of the execution of the order must be prepared in Polish. In the case of documents prepared in a foreign language, The Contractor is obliged to submit a translation into Polish, certified by the Contractor. This procedure is conducted in accordance with the Regulations for the award of public contracts with a value below PLN 170, 000.00 net, constituting Annex No. 1 to Management Board Resolution No. 170/2025 of December 15, 2025. The contractor may submit only one offer in electronic form, with a valuation of the subject of the order described in the request for quotation and within the set deadline. Unfulfillment any of the above conditions will result in the offer being rejected.
