Lokalna 14, in accordance with Annex No. 1 - Description of the Subject of the Order. The contractor is obliged to submit: - an offer form, in accordance with the template of Annex No. 2; - bill of quantities, in accordance with the template of the appendix to the OPZ; - catalog card and/or other documents containing the specification of the product and its physical properties confirming the fulfillment of the parameters specified in point 2.2. b) - h) description of the subject of the order. All documents submitted by the Contractor as part of the order execution must be prepared in Polish. In the case of documents prepared in a foreign language, the Contractor is obliged to submit a translation into that language Polish, certified by the Contractor. This procedure is conducted in accordance with the Regulations for awarding public contracts with a value below PLN 170, 000.00 net, constituting Annex No. 1 to Resolution of the Management Board No. 170/2025 of December 15, 2025. The contractor may submit only one offer in electronic form, with a valuation of the subject of the order described in the request for quotation and within the set deadline.
