Project: PROCUREMENT OF CERTIFICATION SERVICES FOR PSHS SYSTEM-S ISO

9001:2015 CERTIFICATION

Approved Budget for the Contract (ABC): PhP300, 000.00



The PHILIPPINE SCIENCE HIGH SCHOOL SYSTEM - OFFICE OF THE ExECU-TIVE DIRECTOR (PSHSS-OED) intends to apply the sum of THREE HUNDRED THOUSAND PESOS (Php 300, 000.00), being the Approved Budget for the Contract (ABC) to pay for the contract for the Project: PROCUREMENT OF CERTIFICATION SERVICES FOR PSHS SYSTEM-S ISO 9001:2015 CERTIFICATION



1. The PSHSS-OED now invites qualified Filipino-owned consultancy firm / individual to submit price quotations for the above item with the following specifications:



1.1 Should be a Filipino/ Filipino-owned corporation, incorporated under Philippine laws or has entered in a joint venture agreement with Filipino owned company, engaged in QMS



1.2 Duly registered Filipino sole proprietorship and partnership are also invited to apply.



1.3 Mayor-s permit issued by the city or municipality where the principal place of business of the prospective consultant is located;



1.4 Tax clearance per Executive Order 398, Series of 2005, as finally reviewed and approved by the BIR.



1.5 Certifying body must have comprehensive background and trainings in the quality assurance field pertaining to ISO 9001 QMS auditing and certification.



1.6 Statement of the prospective bidder of all its ongoing and completed government and private contracts, including contracts awarded but not yet started, if any, whether similar or not similar in nature and complexity to the contract of this procurement project.



1.7 Audited Financial Statement/ Income Tax Clearance





2. Procurement will be conducted through one of the Alternative Modes of Procurement which is -A"Small Value Procurement-A
