1) The bidders shall submit the following eligibility documents in addition to the prescribed quotation form:

(a) Registration certificate from Securities and Exchange Commission (SEC), Department of Trade and Industry (DTI) for sole proprietorship, or Cooperative Development Authority (CDA) for cooperatives, whichever is applicable;

(b) Mayor''s permit issued by the city or municipality where the principal place of business of the prospective bidder is located;

(c) Certificate of Registration from the BIR;

(d) PhilGEPS Certificate of Registration

(e) Latest Income Tax Return with Financial Statements;

(f) Tax Clearance
2) All entries shall be typed or written in a clearly legible manner;

3) Prices are inclusive of taxes, delivery and other incidental costs;

4) Delivery schedule must be FOB DTI Misamis Oriental, 3F, Antolin Building, Tiano-Akut Streets, Cagayan de Oro City

5) Provide at leas one year warranty on all equipment supplied;

6) Ensure the availability of after sales services and prompt response of not more than five (5) working days from date of Notice of Need
