1.2 Valid certificate or proof of PhilGEPS Registration (Platinum/Red Membership, as applicable under IRR)
1.3 Latest Income Tax Return or Business Tax Return, duly received by the BIR (For ABC-s above Five Hundred Thousand Pesos (PhP500, 000) only)
1.4 Duly notarized Omnibus Sworn Statement (Form No 2) with attached duly notarized Secretary-s Certificate or Special Power of Attorney, whichever is applicable (For
ABC-s above Fifty Thousand Pesos (PhP50, 000.00) only).
Note: A valid PhilGEPS Platinum Registration Certificate may be accepted in lieu of the required eligibility documents, subject to verification and the submission of additional supporting documents, if deemed necessary by the Procuring Entity in accordance with the IRR of RA 12009.
