g. Bill of Quantities;
h. Summary Sheet;
i. Notarized Omnibus Sworn Statement (Must use the GPPB-prescribed template under
RA 12009 (NGPA). Submissions using the old RA 9184 template will be rejected. For
corporations/partnerships, attach a notarized Special Power of Attorney, Board or
Partnership Resolution, or Secretary's Certificate, whichever is applicable); and
j. Performance Security (to be submitted after issuance of Notice of Award).
