ELIGIBILITY AND TECHNICAL COMPONENTS:
A) ELIGIBLITY DOCUMENTS:
Class -œA- Documents:
1. PhilGEPS Certificate of Registration (PLATINUM)
a) DTI Certificate;
b) Mayor-s Permit;
c) Audited Financial Statement; and
d) Tax Clearance.
2. Statement of all On-Going Government & Private Contracts including contracts awarded but not yet started whether similar or not similar in nature and complexity to the contract to be bid
3. Statement of the bidder-s Single Largest Completed Contract (SLCC) similar to the contract to be bid
4. NFCC computation or a committed Line of Credit from a universal or commercial bank.
Class -œB- Document:
5. If applicable, the Joint Venture Agreement (JVA) in case the joint venture is already in existence, or duly notarized statements from all the potential joint venture partners.

INSTRUCTION TO BIDDERS (ITB):
1) Please arrange your documents according to the requirements stated above.
2) Bidders will drop only one (1) big envelope which contains three (3) envelopes namely: ORIGINAL BID, COPY NO. 1 and COPY NO. 2 and each envelope contains the Eligibility and Technical Components and the Financial Components.
3) Envelopes shall be properly sealed and marked:
a) contain the name of the contract to be bid in capital letters;
b) bear the name and address of the Bidder in capital letters;
c) be addressed to the Procuring Entity-s BAC;
d) bear the specific identification of this bidding process; and
e) bear a warning -œDO NOT OPEN BEFORE- the date and time for the opening of bids.
4) Late Bids shall not be accepted.
