Original Receipt obtained from the Shroff of the District General Hospital, Nawalapitiya upon payment of a non-refundable bid fee of Rs. 2, 000.00 for each package along with a copy of the Business Registration Certificate should be submitted to obtain bid forms. Bid documents may be obtained from the Accounts Division of the District General Hospital, Nawalapitiya between 9.30 a.m. and 2.30 p.m. on normal working days from 11.09.2023 until 02.10.2023. If a representative is calling over to collect the documents, a letter of authorization from the applicant and identity card for the identification purposes should be submitted.
