1. The subject of the order is the purchase of an X-ray machine with a C-arm for the Urology Operating Room.2. A detailed description of the subject of the order is included in the Technical and operational parameters form, which constitutes Annex No. 1.1 to the Offer Form.3. Item offered the order must be approved for trading and use in accordance with applicable law, meet the requirements of Polish and European standards, be marked with the CE mark, and have documents issued by authorized entities, required by Polish law (e.g.: CE mark, Declaration of Conformity, necessary certificates, approvals, certificates) in accordance with the Act of April 7, 2022 on medical devices (consolidated text: Journal of Laws of 2022, item 974). At the same time, the Contractor will be obliged to present, at each request of the Ordering Party, appropriate document.4. The technical and operational parameters of the subject of the order are set out in Appendix No. 1.1 to the Offer Form. Failure to meet the required parameters gives the Ordering Party grounds to reject the offer.5. Partial individual deliveries are not allowed elements of the subject of the order.
