1. The subject of the contract is the purchase, delivery and assembly of medical equipment for patient care and therapy for the needs of hospital departments under: Task 1 - project entitled . "Improving the quality and availability of diagnostics and treatment of cancer diseases at the Dr. Ludwik Rydygier Provincial Hospital in Suwałki "co -financed by the National Plan of Reconstruction and the Province Budget in the quantity and assortment specified in Annexes 5 and 9 to SWZ

2. The detachment description of the subject of the contract is included in Annex 5 and 9 to SWZ and in the template of the contract constituting Annex 2 to SWZ.

purchase, Delivery and assembly of medical equipment: Chemotherapy chair in the amount of assortments and with the parameters indicated in the Studznik 5 and 9 to SWZ

Purchase, delivery and assembly of medical equipment: treatment table in the amount of assortments and with the parameters indicated in the attachment No. 5 and 9 to SWZ

part 3

purchase, delivery and assembly of medical equipment: a transport trolley in the amount of assortments and with the parameters indicated in the hazel 5 and 9 to SWZ

part 4

Purchase, delivery and assembly of medical equipment: a set of pumps in the docking station in the number of assortments and with the parameters indicated in the attachment No. 5 and 9 to SWZ
