Vascular Surgery and Angiology in a modern form hybrid room at the University Hospital No. 1 Dr. A. Jurasz in Bydgoszcz.
2. The subject of the order was divided into 2 parts:
part no. 1 - Delivery and installation of an angiographic system and other medical equipment along with modernization and additional equipment of the Vascular Surgery and Angiology Clinic
part No. 2 - Laser.
The offer in each part should be complete and should meet the detailed requirements specified in the SWZ and its annexes.
A detailed description and method of order execution are included in SWZ i annexes, in particular in:
1) Price and product form, constituting Annex No. 1a to the SWZ - applies to parts No. 1 and 2
2) Functional and Utility Program with annexes, constituting Appendix No. 7 to the SWZ - applies to part No. 1
3) Draft provisions of the contract with annexes, constituting Annexes No. 5a and 5b to the SWZ.
