1. The subject of this order is the PURCHASE OF EQUIPMENT FOR THE ANESTHESIA AND INTENSIVE THERAPY DEPARTMENT AT PCK 7 ST. EGO HOSPITAL SPECJALISTOWY IM. OF THE HOLY VIRGIN MARY IN CZESTOCHOWA TO MEET THE EPIDEMIOLOGICAL CHALLENGES AND THE EFFECTS OF THE PANDEMIC.

2. The subject of the order includes 5 parts:

1) intensive care bed,

2) infusion pumps, volumetric pumps, docking station,

3) ventilator,

4) suction unit,

5) kidney therapy apparatusreplacement.

3. The detailed scope of the order is specified in Appendix No. 2 to the ToR - the assortment and price form and in Appendix No. 3 to the ToR - description of the subject of the order. Other conditions regarding the execution of the order are specified in the contract template, constituting Appendix No. 8 to SWZ.
