1. The subject of the order is the purchase, delivery and expansion of IT systems - Integration and expansion of the healthcare provider's IT systems - Expansion of the PACS radiological system, integration of the hospital system to the PUI platform at the Ordering Party's headquarters, i.e.: WSS No. 4 in Bytom, Al. Legionow 10, 41-902 Bytom. 2. The subject is implemented as part of the Project KPOD.07.03-IP.10-0419/25 entitled Accelerating the processes of digital transformation of health care through the development of digital services at the Provincial Specialist Hospital No. 4 in Bytom as part of the investment D1.1.2 "Acceleration of processes digital transformation of health care through further development of digital services in health care. 3. A detailed description of the subject of the order is set out in Annex No. 1A - Assortment and price form, Annex No. 1B - Description of the subject of the order. Information regarding delivery rules and guarantees and remuneration are included in Appendix No. 6 to the SWZ - Draft provisions of the contract.
