The subject of the order is the purchase and successive delivery of drugs from drug programs, specified in parts for the University Hospital Clinical im. Fryderyk Chopin in Rzeszów 2. A detailed description of the subject of the order and the Ordering Party's requirements are specified in the Price and Offer Forms - Annexes No. 1 for individual parts that are an integral part of the SWZ. 3. The ordering party requires offer items of the order that: 3.1. meet the requirements described in the Price and Offer Forms, 3.2. meet the requirements described in the SWZ, 3.3. are covered by the Act of May 12, 2011 on the reimbursement of medicines and special foodstuffs nutritional purposes and medical devices - applies to reimbursed medicinal products. 4 . The scope of the order includes the delivery of order items in the quantities specified in the Price and Offer Forms for the period: Part from No. 1 to No. 4 - 12 months 5. ORDER COMPLETION DEADLINE: 5.1 Purchase and successive delivery of the items of the order: Part no. 1 to no. 4 - for 12 months from the date of conclusion of the contract. 5.2 In Parts No. 1 to No. 4, the Ordering Party allows for the possibility of changing the date validity of the contract concluded with the Contractor in the event of failure to complete the entire subject of the contract during its duration.
