1. The subject of the order is the purchase, delivery, assembly and commissioning of a CT scanner along with the modernization of a room at the Ordering Party's headquarters and the provision of maintenance services during the warranty period for the completed task. 2. Subject implemented is part of Project No. KPOD.07.02-IP.10-0354/25/KPO/13/2025/730 entitled "Modernization of the infrastructure and equipment base of the Provincial Specialist Hospital No. 4 in Bytom to improve access and increase the quality of cardiological care" implemented under the National Reconstruction and Resilience Plan: Component D "Efficiency, accessibility and quality of the health care system", Investment D1.1.1 "Development and modernization of infrastructure of highly specialized care centers and other entities "medicinal" 2. A detailed description of the subject of the order is set out in Annex No. 1A - Assortment and price form, Annex No. 1B - List of parameters and required conditions.
