Tender Details

1. The subject of the order is the purchase and delivery of reagents for protein electrophoresis tests along with the lease of analyzers for medical entities in the Podkarpackie Voivodeship listed in Chapter I SWZ.

2. A detailed description of the subject of the order is included in Annex No. 4 to the SWZ.

3.Execution of the projectThe issuance of the order will be made to the Ordering Parties listed in Chapter I SWZ, in the quantities and assortment indicated in Annex 4 to SWZ.

4. The Ordering Party informs that before initiating the procedure, it conducted preliminary market consultations in accordance with Art. 84 of the Public Procurement Law. Documentation from aboveThe latest market consultations are an annex to the SWZ.

5. The complete offer includes (obligatory):

1) Offer prepared in accordance with the template of Annex No. 1 to the SWZ. Offer form;

2) Completed attachments from No. 4.1 to No. 4.4 listed in Appendix No. 4 to SWZ-SOPZ;

5.1. Other documenty declarations submitted with the offer:
