The subject of the order is: successive delivery to the registered office of the Ordering Party - PHARMACY and/or PHARMACY DEPARTMENT - (including transport and unloading at the premises indicated by the employee room) of products listed in terms of assortment and quantity in the assortment and price form (sets 1, items 1 to 5) constituting Annex No. 2 to the SWZ. 2. A detailed description of the subject of the order and the terms of implementation are specified in: a) offer form, which constitutes Annex No. 1 to the SWZ, b) the detailed scope of the subject of the order is specified in the assortment and price form (set no. 1, items 1 to 5), which constitutes appendix no. 2 to the SWZ, c) draft contract, the template of which is Annex 8 to the SWZ. 3.Place delivery will be: a. Karola Miarki 14, 46-082 Buy - premises of the Hospital Pharmacy - 3rd floor of the main building of the Hospital in Kup, b.Rehabilitation Hospital in Pokoju, ul. Namysłowska 22, 46-034 Room - Department premises Pharmacy - the main building of the Hospital in Peace. 4. Each order of medicines will indicate the place of delivery, i.e. delivery to the HOSPITAL PHARMACY in Kup, or delivery to the PHARMACY DEPARTMENT in Pokoju. 5.
