Task No. 1 - Hemostatic and bactericidal dressings Task No. 2 - Sterile dressings Task no. 3 - Care balm Task No. 4 - Hydrogel and Manuka honey dressings 1.1. The subject of the order is the successive deliveries of dressing materials. 2.Description the subject of the order is included in Annex No. 2.1 - 2.4 to the SWZ (assortment and price form) (part of the order/package/task no. 1 - 4) - also collectively referred to as "Annex No. 2 to the SWZ". 3. Place of order fulfillment: ul. Grenadierów 51/59, 04-073 Warsaw. 4. Successive deliveries of the subject of the order are to be made to the Hospital Pharmacy located in Pavilion III of the Hospital on the ground floor and to the warehouse located in the basement of the Hospital, and each time the Ordering Party will indicate if the place of delivery will be the warehouse. 5. The subject of the order will be collected in rooms available to the Contractor's employees (suppliers, couriers). 6. Detailed conditions for the provision of the service are specified in the draft contract constituting Annex No. 4 to SWZ. 7. The Ordering Party allows the possibility of submitting partial offers (4. - tasks / parts of the order), in accordance with the division specified in Annex No. 2 to the SWZ constituting the Description of the Subject of the Order form: 1) task no. 1 - Hemostatic dressings i bactericidal; 2) task no. 2 - Sterile dressings; 3) task no. 3 - Care balm; 4) task no. 4 - Hydrogel and Manuka honey dressings. 8.
