Kilińskiego 25 in Sosnowiec as part of the implemented project "Get a profession and qualifications - transformation of CKZiU ul. Kilińskiego 25". STATEMENTS AND DOCUMENTS SUBMITTED WITH THE OFFER. 1.1. The offer is submitted in electronic form (i.e. with a qualified electronic signature) under pain of invalidity. The offer is due prepared in accordance with the template provided in Annex No. 1 to the SWZ. 1.2. The Contractor attaches to the offer a declaration of not being subject to exclusion to the extent indicated in Chapter III, section 2 SWZ: a) point 2.2.
