- SUPPLIER WHO FAILED TO SUBMIT ELIGIBILITY DOCUMENTS WILL NOT BE INCLUDED IN THE EVALUATION, NOTARIZED OMNIBUS SWORN STATEMENT (OSS) SHALL BE SUBMITTED BY THE WINNING BIDDER.
