Description : TERMS and Conditions:

1. Prices and specifications for the quotation should be valid for 45 days.

2. Bid should not exceed the Approved Budget for the Contract.

3. DELIVERY PERIOD: 30 calendar days upon receipt of the Purchase Order.

4. The price/s quoted must include all kinds of taxes and other charges.

5. Late Quotation/s will not be accepted and Quotation/s sent thru e-mail will not be entertained.

6. Award of contract shall be made to the bidder/supplier with TOTAL LOWEST RESPONSIVE QUOTATION that complies with all the requirements hereof.

7. a.) Mayor-s/Business Permit, b.) Platinum PhilGEPS Registration c.) BIR Registration, d) Tax Clearance and e) Duly notarized Omnibus Sworn Statement shall be submitted together with the price quotation. Non-submission shall be a ground for rejection of the quotation.

8. Any alteration or erasure must be initialed by the person who signs the quotation.

9. Liquidated damages equivalent to one tenth of one percent (0.001%) of the value of the goods not delivered within the prescribed delivery period shall be imposed per day of delay.
