The subject of the order is the provision of services for leasing domestic WAN lines, hereinafter referred to as the "service". 3. A detailed description of the subject of the order is attached as Annex No. 5 to the SWZ and Chapter XIX SWZ. 4. The Ordering Party does not require the submission of the said evidence along with the offer. 5. Employment requirements based on an employment relationship in the circumstances referred to in Art. 95 of the Act: 1) The Ordering Party requires employment by the Contractor or a Subcontractor based on an employment relationship of persons performing the following activities in the scope of order execution: supervision over the proper functioning of connections. 2) during the execution of the order, the Ordering Party is entitled to perform activities inspections towards the Contractor regarding compliance by the Contractor or Subcontractor with the requirement of employing persons performing the activities indicated in point 1 on the basis of an employment relationship. In order to verify the fulfillment of these requirements, the Ordering Party is entitled to in particular to request: a) declarations of the employed employee, b) declarations of the contractor or subcontractor about employing an employee under an employment contract, c) a certified true copy of the employment contract...
