The subject of the order is the provision of health services. The provision of health services will include the provision of: a) as part of the basic order - for employees of the National Fund for Environmental Protection and Water Management and retirees and pensioners (former employees of the National Fund for Environmental Protection and Water Management). As part of the order basic benefit is expected to include: - employees, with the average monthly employment being 842 people, - Members of the Management Board - 5 people, as well as - retirees and pensioners, where the monthly average is determined the number of retirees and pensioners at the level of 160 people. b) under the option right - for family members of NFOŚiGW employees and members of the Management Board as well as family members of retirees and pensioners (former NFOŚiGW employees). Under option law, an average is expected the monthly number of family packages at 371, the number of people covered by the senior package - 90, and 15 people covered by the family package for members of the Management Board. The ordering party will have the right to use the so-called option rights within 36 months from the date of signing the contract, providing the Contractor with an appropriate declaration. A detailed description of the subject of the order is presented in Annex No. 4 to the SWZ.
