Tender Details

1. The subject of this order is the provision of services in the field of testing for sanitary and epidemiological purposes of students of the University of Warmia and Mazury in Olsztyn.2. As part of the service that is the subject of the order, the Contractor will be obliged to: perform medical examinations and additional tests (chest X-ray, USR and others as indicated by the doctor) for sanitary and epidemiological purposes of the above-mentioned students. Faculty of the University of Warsaw in Olsztyn.3. In the case of the tests described in point 2 based on medical examination i results of laboratory tests, the doctor issues a medical certificate for the student stating that there is no contraindication, temporary or permanent contraindication to work or study, documenting the result in the test card for sanitary and epidemiological purposes. Results The doctor enters the laboratory tests performed and the content of the certificate in the test book for sanitary and epidemiological purposes and forwards it to the student.
