The Ordering Party invites you to participate in the procedure in the basic mode in variant 2 for: "Provision of reservation and sale services of accommodation in hotels or other accommodation for the needs of the Institute of Mathematics of the Polish Academy of Sciences" pursuant to art. 275 point 2 of the Public Procurement Law. The conditions for participation in the procedure and submission of offers are specified in the SPECIFICATION OF ORDER CONDITIONS (SWZ) located in the Attachments tab. NOTE! 1. The offer must be prepared in electronic form under pain of invalidity. 2. Contractor is obliged to submit the following documents and declarations signed by authorized persons via the Purchasing Platform in the "Appendices" tab: 1) Offer in accordance with the content of the Offer Form, constituting Annex No. 3 to the SWZ and the list of services constituting Annex No. 4 to the SWZ;2) Declarations constituting Annexes No. 5-7 to the SWZ3) Power of attorney/powers of attorney on the terms described in the SWZ in a form consistent with the regulation on the method of preparing and transmitting information (if applies);4) Declaration regarding the division of responsibilities between contractors, in accordance with point 11.3.
