1. The subject of the order is the supply of sensors for measuring the depth of anesthesia together with the rental of sleep depth monitors to the headquarters of the Ordering Party (Kraków, os. Zlotej Jesieni 1), in accordance with the description and requirements contained in Annex No. 1A and Annex No. 1B hereto. specifications.2. The expiry date/warranty period of anesthesia depth sensors cannot be shorter than 12 months from the date of delivery.3. Requirements regarding sensors for measuring the depth of anesthesia: a) The Contractor undertakes to carry out the Ordering Party's request for training of the Ordering Party's staff at its headquarters in Os. Zotej Jesieni 1, Krakow with the first delivery of the goods to the Szpitalna Pharmacy within the time agreed in writing with the Ordering Party, but no later than 30 days from the date of conclusion of the contract. contract. b) The contractor will be obliged to submit an attendance list from the training referred to in section 3 points a) with signatures of trained users confirming their actual presence to the Hospital Pharmacy, within 5 business days from the date of training. 4.
