1. The subject of the order is the provision of 920 hours of services of a psychologist conducting consultations for dependent persons, seniors and informal caregivers in a designated place in the Stary Dzierzgo Commune, Pomeranian Voivodeship, including: a) Psychologist's service provided at the place of residence of dependent persons, i.e. seniors, people with disabilities, single people, chronically ill: Number of people supported: minimum 20; Number of hours per 1 project participant: on average 22 hours (on average 1.5 h/month) Number of hours of service covered by the contracts: 440 h (one hour = 60 minutes). The specialist service will be provided with transport to the individual places of residence of dependent persons, i.e. in the area of the Stary Dzierzgo commune, from Monday to Friday on days from 8.00 a.m. to 5.00 p.m. Distribution individual journeys to destinations will vary, depending on the recruited Participants. b) Psychologist service as part of group and individual support, provided at the Day Care Home in Stare Dzierzgoń for seniors and people with disabilities disabilities: - Number of people supported: minimum 15; - Number of hours of group support: 1 meeting per month, 2.5 hours; - Number of...
