The subject of the contract is a task aimed at increasing the opportunities of people with disabilities to lead a more independent, self-reliant and active life and striving to improve their functioning in the environment, increase the opportunities meeting needs and including them in social life. The implementation of the task is co-financed by the Solidarity Fund. The order must be performed in accordance with the rules set out in the Program of the Minister of Family, Labor and Social Policy "Personal assistant for a person with a disability" for Local Government Units - 2026 edition (hereinafter referred to as the "Program"), which constitutes Annex No. 7 to the SWZ. Assistance services will be provided to residents of Ruda Śląska, including children (hereinafter referred to as Program Participants), indicated to the Contractor by the Ordering Party. Expected, estimated number of people using the services: 33 people (3 children between 2 and 16 years of age and 30 people with a certificate of significant disability or equivalent, including 14 people with multiple disabilities). Total number of planned hours of assistant services: 23, 519 of which 1, 080 for children.
