Subject of the contract
1. The subject of the contract is the supply of endoprosthesis and audit endoprosthesis of the hip to the headquarters of the Ordering Party (Ludwik Rydygier Specialist Hospital in Krakow sp.z o.o., Os. Golden Autumn 1, 31-826 Krakow), in accordance with the description and requirements contained in the attachment No. 1a and 1b to the specification (in terms of the part for which the offer is submitted).
24 months from the date of delivery.
3. Additional conditions regarding the subject of the contract:
a) The Contractor undertakes to deliver and deposit free of charge at the headquarters of the Ordering Party (Krakow, Os. Złota Autumn 1) subject of the contract together with instruments for the duration of the contract. The Contractor will be obliged to freely supplement and service the instrumentation provided to the Employer under the contract. The contractor provides a minimum of 2 sets of implants, two sets of tools and Motor kit plus blades to choose from by the contracting authority. The contractor provides preoperative planning software (applies to package 1).
b) The contractor will be obliged to transfer the instrumentation along with its list or photos of tools.
4. In the case of offering new products not used so far at the Ordering Party, the Contractor will be obliged to freely train the Employer's personnel at the headquarters of the Ordering Party (Krakow, Golden Autumn os. confirm ...
