1. The subject of the order is the provision of comprehensive maintenance services for the CliniNET and NetRaad IT systems at the Institute of Tuberculosis and Pulmonary Diseases in Warsaw, for which the Institute has an indefinite license.2. Detailed description of the subject of the order, requirements and the order quantities were specified in: Annex No. 11 - Description of the scope of maintenance services, Annex No. 10 - List of functions of the modules and in Annex No. 9 - Description of the minimum requirements of the infrastructure on which the System will be run, constituting an annex to the SWZ. A detailed description of the subject of the order is provided in the Price Form constituting Appendix No. 4 to the SWZ and the contract template. The completed Price Form is submitted with the Offers.
