The subject of the order is the provision of care services and physiotherapy services to participants of the Pomocna Do project - a comprehensive long-term care program for residents of the City of Szczecin FEPZ.06.21-IP.01-0022/25 co-financed by the European Social Fund Plus under the European Funds for Western Pomerania program. The project participants live in the City of Szczecin. The following services will be provided as part of the order: 1) care services for 10 project participants in their place of residence. the services will be provided on the basis of an individual support plan developed for each participant with an average of 12 hours per month per participant. 2) physiotherapy services for 10 project participants in their place of residence. services will be provided on the basis of a plan developed for each individual participant support plan with an average of 8 hours per month per participant.
