TERMS OF REFERENCE
DENTIST`S SERVICES
I. QUALIFICATION:
"Must be a Doctor of Dental Medicine.
" Must have at least (4) years of relevant experience.
" Must have a current PTR.
" The license must not be expired.
II. WORK STATION:
The Dentist must hold clinic in MCWD Clinic, 2nd Floor, MCWD Main Bldg.
III. WORK HOURS:
The Dentist must render six (6) hours a day, 5 days a week or (120) hours during the month with two (2) days off.
MONDAY TO FRIDAY NOON BREAK
8:00AM - 12:00PM 12:00PM - 1:00PM
1:00PM - 3:00PM
IV. SCOPE OF SERVICES:
A. Renders the following services to the employees free of charge:
1. Annual Dental Check-up
2. Annual Oral Prophylaxis
3. Gum Treatment
4. Gingivitis Treatment
5. Oral Consultation
6. Temporary filling (anterior or posterior tooth)
7. Extraction: Anterior/Posterior tooth
8. Drainage Abscess
B. Renders other services to the employees at the minimal cost (see attached sheet)
C. Renders the following services to the dependents free of charge:
1. Annual Oral Prophylaxis
2. Gum Treatment
3. Gingivitis Treatment
4. Oral Consultation
5. Dental Check-up
6. Temporary Filling
7. Simple Tooth Extraction (Anterior Tooth Only)
8. Deciduous Tooth Extraction (Anterior or Posterior for children aged 7 to 12)
V. LENGTH OF SERVICE:
The company dentist retainership will cover a thirty (30) month period, renewable after 2 and a half years or an equivalent total sum of 360, 000.00 only.
VI. RETAINERS FEE:
The retainer`s fee of 12, 000.00/mon. inclusive of tax will be paid out to the Dentist Retainer and a taxable amount will also be withheld on other income specified under the Dental Services at a discounted rates.
VII. SPECIFIC PROVISIONS:
In case of absence, the dentist must inform in advance so the patient/employee will be advised to see other dentist. However, for leave of absence that will exceed 1 week, he/she shall appoint a temporary dentist no cost to MCWD.
VIII. OBSERVANCE OF EXISTING MEDICAL/DENTAL GUIDELINES
Dental Consultations and reimbursements will be based on the approved rates.
IX. REPEALING CLAUSE:
Those provisions in the existing guidelines which are inconsistent with the foregoing provisions are hereby deemed repealed.
