1. Valid Registration Certification from SEC/DTI or CDA

2. Valid Mayor-s Permit

3. PhilGEPS Registration if Platinum or screenshot of PhilGEPS registration if not on a Platinum status

4. Notarized Omnibus Sworn Statement and signed by Authorized Representative with correct project title

5. Properly filled up and signed RFQ

6. Quotation with unit cost per item. Any item exceeding the unit cost in the PR shall automatically be DISQUALIFIED...
